Who do you call for a Tubal Liation... Gettng your tubes tied?

My husband and I are trying to decide wether to get my tubes tied, or have him get a vasectomy. I know that it is cheaper for him to get a vasectomy, but does the same go with health insurance? Who do I call about this? Also, who do I call to make an appt. for either procedure? The hospital, my gynocologist, our family doctor, a doctor for him???? Please help, I don't even know where to start.
